Owner, Physical Therapist
Dr. Michael Jones PT, DPT, OCS, ATC, CSCS
Dr. Michael Jones has been helping individuals achieve their goals for more than 10 years. He began his career as an Athletic Trainer at University of California Berkeley with both men’s and women’s athletics. From there, he decided to specialize in rehabilitation and moved home to attain his Doctorate in Physical Therapy at Nova Southeastern University. As a young clinician, he further distinguished himself by becoming a Board Certified Orthopedic Clinical Specialist.
Through years of practice, Dr. Jones has gained extensive experience rehabilitating and training a wide variety of clients. From professional athletes in the NFL, MLB, and MLS to homebound individuals that experienced a recent hospitalization due to stroke, fall, or other health concerns. Regardless of your current physical status, Dr. Jones will help you achieve your goals!

Physical Therapist, Office Administrator
Dr. Jenna Jones PT, DPT, NCS, CSCS
Dr. Jenna Jones is a Physical Therapist and Board Certified Neurologic Clinical Specialist. She began her career by completing a Neurologic residency with the University of Miami and Jackson Memorial Hospital in Miami, Florida. Her experience has spanned from working with mechanically ventilated patients in the trauma ICU to working in an outpatient setting with retired NFL players diagnosed with Parkinson's disease and post-concussion syndrome with many diagnoses and ability levels in between.
Her passion is helping people with Neurologic disorders return to their highest level of function and activities that they love. In her free time she enjoys spending time with her family, going to the beach, traveling and working out!

Physical Therapist
Dr. Aubrey Hall PT, DPT
Dr. Aubrey Hall is a licensed Physical therapist that has been helping patients achieve their functional goals for the past 5 years. He obtained a Bachelor’s in Exercise Science from Troy University and his Doctorate in Physical Therapy from Samford University. In his spare time, Aubrey enjoys volunteering at his church as a youth leader for youth groups and teens. Aubrey began his career as an outpatient physical therapist treating a wide range of patients from pediatric to geriatric, all obtaining orthopedic related injuries. He then moved over to become an inpatient physical therapist specializing in trauma related injuries, various abdominal surgeries, and joint replacements. No matter what your current physical status or goal is, Dr. Hall can help you achieve your goals of returning your functional independence.

Physical Therapist
Dr. Brendan Bak PT, DPT
Born and raised in Jacksonville, Dr. Brendan Bak has been a physical therapist serving the local community for over 5 years. Dr. Bak received his Bachelors of Science from the University of North Florida and immediately began to pursue a career in physical therapy. After receiving his Doctorate in Physical Therapy from Nova Southeastern, Dr. Bak returned to Jacksonville and immediately began working in an orthopedic/sports outpatient setting. Three years in to practicing, Dr. Bak furthered his specialization in orthopedics by completing a 1 year long orthopedic residency program with the Brooks Institute of Higher Learning.
In addition to his advanced training in orthopedics, Dr. Bak enjoys working with geriatric patients to improve their gait and balance so that they can confidently do their day to day tasks and have a higher quality of life. Dr. Bak will help you address your musculoskeletal concerns and help reach your goals!

Occupational Therapist
Sarah Conklin OTR/L, CLT
Sarah is a dedicated female occupational therapist with 8 years of experience in acute care, stroke rehabilitation, and upper extremity (UE) rehabilitation. With a strong passion for working with the elderly, Sarah has honed her expertise in helping individuals regain their independence and improve their quality of life. Spending the last several years working in acute care therapy and transitioning to home health therapy, she is excited to provide more personalized care and extended time to better support her patients' rehabilitation journeys.
